What Is Manual Therapy?
Manual therapy is a specialized form of physical therapy that uses precise hands-on techniques to restore joint mobility, release muscle tension, and alleviate pain. Unlike traditional approaches that may only address symptoms temporarily, manual therapy targets the mechanical sources of dysfunction—promoting long-term healing and improved movement patterns.
At Pyramid Physical Therapy & Pilates, Dr. Cynthia Mendez, PT, OCS, DPT, combines advanced manual therapy expertise with personalized therapeutic exercises to help San Antonio residents achieve lasting relief from chronic and acute pain conditions.
Who Benefits from Manual Therapy in San Antonio, TX?
Manual therapy is effective for a wide range of conditions affecting mobility and comfort:
- Chronic low back and neck pain
- Joint stiffness from injury or arthritis
- Neurogenic claudication (nerve-related leg pain)
- Myofascial trigger points (muscle knots)
- Postural dysfunction causing pain
- Muscular imbalances leading to compensation patterns
- Post-surgical restrictions limiting movement
- Sports injuries affecting performance
Whether you’re recovering from an acute injury, managing a persistent condition, or seeking to prevent future problems, manual therapy provides a foundation for sustained improvement.
Key Benefits of Manual Therapy
Restores Joint Mobility
Manual therapy techniques gently guide restricted joints through their full range of motion, releasing adhesions and improving flexibility. This enhances overall body coordination and reduces strain on surrounding muscles.
Alleviates Acute and Chronic Pain
By addressing nerve compression, muscle tension, and joint dysfunction, manual therapy helps reset your body’s pain signals. This allows you to return to previously painful activities with confidence and reduced discomfort.
Improves Movement Patterns
Our therapists work to reestablish efficient biomechanical patterns through controlled movement challenges. This strengthens your body’s natural stabilization systems and promotes fluid, coordinated motion.
Builds Long-Term Tissue Capacity
Consistent manual therapy progressively expands your baseline strength, flexibility, and neuromuscular control—helping you participate in daily activities and recreation with minimal risk of re-injury.

Gentle sustained pressure releases rigid fascia and scar tissue, restoring muscle elasticity and reducing pain.

Frequently Asked Questions About Manual Therapy in San Antonio
What’s the difference between manual therapy and massage therapy?
While massage applies general pressure for relaxation, manual therapy uses specific techniques performed by licensed physical therapists to correct joint restrictions and muscle dysfunction related to pain and movement limitations.
Does manual therapy hurt?
Manual therapy should not cause significant pain. Our therapists work within comfortable ranges and adjust pressure based on your feedback. Some temporary soreness similar to post-workout muscle fatigue may occur as tissues adapt.
How many manual therapy sessions will I need?
Treatment duration varies based on your condition’s severity and chronicity. Many patients notice improvements within 2-4 weeks, with full recovery typically requiring 6-12 weeks of consistent care.
Can I do manual therapy without a doctor’s referral in Texas?
Yes! Texas offers direct access to physical therapy, allowing you to see our licensed therapists for evaluation and treatment for up to 10 consecutive business days without a physician referral. If further treatment is needed, we’ll help coordinate with your doctor.
Will my insurance cover manual therapy?

I didn’t know what to expect, being my first time with PT. The facility was clean, equipment modern and in very good condition and the staff were over-the-top Amazing! Everyone there, both patients and therapists were all having fun, laughing and joking. I’m 100% pain free after completing my therapy regimen. An incredible experience that I would recommend to everyone.

Pyramid Plaza is the best physical therapy place I could have ever found. I was at the end of my journey on PT and nothing was working. I decided that I would try something different. Everyone there was so nice, so knowledgeable, and they take the time to really know your situation and your issues and give you a plan made for your body. I cannot recommend Pyramid Plaza more. I usually see Cyndi or Fiorela, and have on occasion seen Bitsy. They are all fabulous!! What are you waiting for, go in, it will make you feel so much better….but you have to take the time to do the work as well.
