Before your first visit.
Paperwork is the part nobody wants to think about, so we do most of it for you. Here is exactly what to bring, what we sort out with your insurer before you arrive, and who to call when something does not add up.
Six things, and one of them is clothes.
Bring what you have. If something on this list is missing, come anyway: nothing here is a reason to delay a first visit.
A photo ID
Driver's license, state ID, military ID. We match it to what your insurer has on file so a name mismatch does not stall a claim later.
Every insurance card you carry
Primary and secondary both. If you have Medicare Part B plus a supplement, bring both cards: the coordination between them affects what gets billed.
A referral or prescription, if you have one
You do not need one to start. Texas direct access lets a licensed physical therapist see you without a referral for up to 10 consecutive business days. If a physician already wrote one, bring it and we will use it.
Imaging and surgical records
X-ray, MRI or CT reports, operative notes, and any post-surgical protocol your surgeon gave you. The report matters more than the disc. It tells your therapist what has already been ruled out.
Your current medication list
Including anything for pain, blood pressure, or blood thinning. It changes how we pace hands-on work and what we watch for during exercise.
Clothes you can move in
Shorts or loose pants for a hip, knee, ankle or back problem. A t-shirt or tank top for a shoulder, neck or arm problem. Supportive shoes if walking or balance is part of why you are coming in.
What Andrea checks while you are still at home.
Andrea Wall is our Patient Care Coordinator. She is not clinical staff, she is the person who makes sure the insurance side is already answered by the time you walk in. Dr. Cynthia Mendez has said plainly what frustrates her about this profession: "Having to jump over hoops when dealing with insurance." Andrea is how we keep those hoops off you.
Your benefits, on your actual plan
Being in-network with your carrier is not the same as knowing what your specific plan does. Andrea calls and confirms your coverage for outpatient physical therapy, what your plan requires you to pay, and whether any visit limits apply, before your first appointment rather than after.
Whether your plan wants a referral
Texas law does not require one for the first 10 consecutive business days. Some plans still ask for one before they will pay, and a few require prior authorization. Andrea finds out which category you are in and tells you, along with the fastest way to get what is missing.
Your appointment, and the paperwork itself
Scheduling, rescheduling and payment all run through Andrea, so one phone call handles the whole thing. She also prepares your intake paperwork, which means you are filling in blanks rather than starting from a blank page.

This is an insurance-based practice. These are the carriers we are in-network with, and Andrea verifies your individual plan against them before you come in.
