Pyramid Physical Therapy & Pilates is a single-location outpatient orthopedic practice at 4939 De Zavala Rd, Suite 103 in San Antonio. Our patients run from weekend warriors with nagging pain to adults working hard to stay independent, and a lot of them arrive having already been treated one body part at a time somewhere else.

The physical therapist here is the person who evaluates, reasons through the problem, and owns the plan of care from the first visit to discharge. You are not writing a program for someone else to run and then hearing about it later. You keep the patient, you see what changed between visits, and you adjust it yourself.

Evaluate before you treat

Take the initial evaluation yourself. Watch how the person walks, reaches, loads and shifts, then build the working diagnosis from what you see, not from the referral line alone.

Own the plan of care

Set the goals, write the plan, and revise it as the patient responds. Our patients are frequently managing more than one region at once, and the plan is expected to account for all of it.

Treat with your hands

Manual therapy is part of the work here, not an upsell: joint mobilization, soft tissue work, and the hands-on reassessment that tells you whether it moved the needle.

Build it back with Pilates

Apply Pilates principles to rehabilitation on the reformer, the tower and the mat, scaled to whoever is in front of you. This is the part of the plan that has to hold after discharge.

Direct the assistant team

Provide the supervision, direction and progression our physical therapist assistants work from, and stay genuinely involved in the patients they carry.

Document, and speak up to referrers

Clean, timely notes, plus real communication with referring physicians. Our owner names one specific frustration with the system: patients not being heard by their referring MDs about what they need. Closing that gap is part of this job.

Texas PT licensure

A current Texas physical therapist license, or eligibility for one, before your first patient. New graduates are welcome to apply.

An orthopedic foundation

Comfort with a general outpatient orthopedic caseload: spine, shoulder, hip, knee, foot and ankle, plus rehabilitation before and after surgery.

Hands-on skill

You should be confident assessing and treating with your hands, and comfortable being watched doing it, because the room is one-on-one.

Real interest in Pilates-based rehab

A Pilates certification is not required to start. Genuine interest is, because the method runs through the plan of care here. Dr. Cynthia Mendez certified as a Pilates instructor in 2008 through Body Arts and Science International.

An attentive listener

Patience with complex cases

A large share of our patients are managing arthritis, chronic pain, balance problems, or recovery from hip and knee replacement. Patience is a clinical skill in this building.

A practice somebody chose to save.

In 2020 the pandemic put this clinic on the verge of closing. Dr. Cynthia Mendez, PT, DPT, OCS had been here since 2006, first as its primary physical therapist and later as Clinical Director, and she was not willing to watch the reputation those years built disappear. She bought the practice in 2021. Almost everything about how this place runs traces back to that decision.
