Privacy policy.

A plain-language account of what this website collects, who touches it on the way to us, and what we will never do with it. Last updated August 16, 2026.

We collect what we need to call you back. Nothing else.

This website exists so people in San Antonio can find us and ask for an appointment. The only information it gathers is what you type into the appointment form yourself. There are no advertising trackers on this site, no ad pixels, and no third-party analytics. We do not sell, rent, or trade your information, and we never will.

The longer version follows, because "we value your privacy" is not a policy.

1. Who this policy is from

Pyramid Physical Therapy & Pilates, an outpatient physical therapy clinic at 4939 De Zavala Rd, Suite 103, San Antonio, TX 78249. This policy covers the website at pyramidpt.com. It does not cover the clinical records we keep about you as a patient, which are governed separately under HIPAA and by the Notice of Privacy Practices you receive at the clinic.

2. What the appointment form collects

Every booking and contact form on this site collects the same short set of fields, and all of them come from you:

  • Your name.
  • Your phone number.
  • Your email address.
  • A short free-text line describing what brings you in.
  • Your preferred callback window, and on the appointment page, your insurance carrier and whether you already have a referral.
  • A hidden field recording which page the form was sent from, so we know where you were reading when you decided to reach out.

Nothing on this site is required reading before you can browse it. If you never submit a form, we never learn anything about you from this website.

3. Where your submission goes

This site is hosted on Netlify, and form submissions are processed by Netlify's form handling service before they reach us. That means your submission passes through Netlify's infrastructure on its way to the clinic and is stored there as part of that service. Netlify acts as our hosting and form processing vendor, not as an audience for your information.

From there, the submission is forwarded into the clinic's practice management system so that Andrea Wall, our Patient Care Coordinator, can call you back, confirm a time, and verify your insurance benefits. Access to that system is limited to clinic staff who need it to do their jobs.

4. Please do not send us medical details through this website

This matters more than anything else on this page. The forms on this website and our general email address are ordinary internet channels. They are not encrypted end to end, they are not a patient portal, and they are not an appropriate place for protected health information.

A short line like "left knee pain since June" is fine and helps us route your call. Diagnoses, imaging results, medication lists, insurance member numbers, dates of birth, and anything you would not want read over someone's shoulder should not go into a web form or an email. Pyramid Physical Therapy & Pilates is a covered healthcare provider, and the health information we hold about you as a patient is protected under HIPAA. Information you volunteer through this website before you are a patient is treated with care and kept confidential, but a public web form is not the protected channel, and we would rather tell you that plainly than let you assume otherwise.

5. Third parties actually present on this site

Three outside services load in your browser when you visit certain pages here. Each one can see your IP address and basic request information simply because your browser has to connect to them, and each has its own privacy policy that we do not control:

  • Google Fonts. Our two typefaces are served from Google's font service on every page.
  • Google Maps. An embedded map appears on pages that show you how to find the clinic, including the appointment request page.
  • Elfsight. Our Google reviews feed is displayed through an Elfsight widget on the homepage and the testimonials page. It reads our public Google Business Profile reviews. It does not read anything about you.

These embeds may set their own cookies or use browser storage when they load. The website itself sets no cookies of its own and runs no advertising, remarketing, or profiling scripts. If you would rather not load the embeds at all, most browsers and content blockers can stop them, and the rest of the site will still work.

6. What we do not do

We do not sell your information. We do not rent or trade it. We do not run advertising pixels, build audience lists from visitors, or hand your details to a data broker. We do not send marketing email to people who filled in an appointment form: if you hear from us, it is about your appointment. And we make no claim to any privacy certification or seal, because we hold none.

7. When we would share something

Only in narrow cases: with the vendors described above, strictly so they can perform the service we hired them for; with your insurance carrier when you have asked us to verify or bill your benefits; and where the law requires it, such as a valid subpoena, court order, or regulatory obligation. That is the whole list.

8. How long we keep it

Appointment requests are kept as long as they are useful for scheduling and for our own business records, and are deleted or archived after that. If you become a patient, your clinical record is kept for the period Texas law and our professional obligations require, which is a separate and longer timeline than a web form submission.

9. Asking us about your information

You can ask what we hold from a form you submitted, ask us to correct it, or ask us to delete it. Call (210) 616-0629 during clinic hours, Monday through Friday, 8:00am to 5:00pm, or email pyramidptandpilates@gmail.com with a short note asking us to get in touch. Do not include medical details in that email. We will confirm who you are before we act on a request, because acting on an unverified one would be its own privacy problem.

Requests about your clinical record as a patient are handled under HIPAA rather than this policy, and the clinic will walk you through that process.

10. Children

This website is intended for adults. We do not knowingly collect information from children through it. A parent or guardian should submit the form on a minor's behalf. If you believe a child has sent us information through this site, call us and we will remove it.

11. Links to other sites

We link out to our Facebook, Instagram, YouTube, and Google Business Profile pages, and to Google Maps for directions. Once you follow one of those links you are on someone else's site, under their policy, and this one no longer applies.

12. Changes to this policy

If we add a tool to this website that changes what gets collected, we update this page and change the date at the top. There is no version history to dig through: what is on this page is what is true today.
