Pyramid Physical Therapy & Pilates is one outpatient orthopedic clinic at 4939 De Zavala Rd, Suite 103 in San Antonio. Our physical therapist assistants are not floating between four patients and a stack of hot packs. They take a patient, work the plan, and stay in the room.

You work under the direction of Dr. Cynthia Mendez, PT, DPT, OCS, our owner and the practice’s physical therapist. She writes the plan of care. You carry it out, you progress it inside the boundaries she sets, and you tell her what you saw so the next session is better than the last.

Six responsibilities, all of them on the floor.

Almost none of this happens at a desk. The job is treating people, watching them closely, and telling the truth about what you saw.

Carry the treatment session

Run the visit start to finish inside the established plan of care: warm-up, hands-on work within your scope, therapeutic exercise, and the wind-down that sends someone home feeling better than they arrived.

Progress the exercise plan

Advance load, range and complexity within the parameters the physical therapist set, and hold the line when the patient is not ready for the next step yet.

Coach on the Pilates equipment

Set springs, position the patient, and cue the movement on reformer, tower and mat. Abdominal and trunk strengthening runs through most of our plans, and the equipment is how it gets loaded safely.

Watch, and report what changed

You are the person who sees the compensation appear at rep eight. Feed that back to the supervising therapist so the plan of care gets revised on real information.

Teach the home program

Send people home knowing exactly what to do and why it matters. An adult who understands their program is far more likely to still be doing it in week four.

Document, and reset the room

Timely notes that reflect what actually happened, and a clinic that is ready for the next patient. Everybody here resets equipment. Nobody is above it.

Six things that actually matter here.

Texas PTA licensure

A current Texas physical therapist assistant license, or eligibility for one, before your first patient. Recent graduates are welcome to apply.

Outpatient orthopedic comfort

Spine, shoulder, hip, knee, foot and ankle, plus rehabilitation before and after surgery. Our team came in through programs like St. Philip’s and Concorde Career College.

Willingness to learn Pilates-based rehab

You do not need to arrive certified. Bethany Roberts, PTA, added her Nationally Certified Pilates Teacher credential after she was already working as an assistant. What we need is somebody who wants to learn the method properly.

Cueing that lands

Clear hands and clear words. A patient who cannot feel the muscle you are asking for needs a better cue, not a louder one.

Working under direction, gladly

The plan of care belongs to the supervising physical therapist. The best assistants push back with observations, not with improvisation.

Steadiness with complex cases

Many of our patients are managing arthritis, chronic pain, balance problems, or recovery from hip and knee replacement. Some are fearful. Steadiness is the skill.

The assistants here are the reason patients stay.

The pandemic put this clinic on the verge of closing in 2020. Dr. Cynthia Mendez, PT, DPT, OCS had been part of it since 2006 and bought the practice in 2021 rather than let it disappear.
